Lateral calcaneal lengthening osteotomy (LCLOT) is an effective procedure for the three-dimensional correction of painful flexible flatfoot (PCFD). Unlike the Evans osteotomy or Z-shaped calcaneal osteotomy, LCLOT is performed at the sinus tarsi, directly addressing pathological hindfoot valgus, foot abduction, and medial arch collapse. This article provides an overview of different calcaneal lengthening osteotomy techniques, exploring their anatomical, biomechanical, and clinical foundations. With high union rates and promising clinical outcomes, LCLOT proves to be a powerful single-site osteotomy for the treatment of PCFD.
