In 1875, Wagstaffe first described avulsion fractures of the distal fibula associated with ankle injuries. A specific type, the Type II fracture, combines an avulsion fracture of the anterior inferior tibiofibular ligament with a fibula fracture, which, if left untreated, can result in syndesmotic instability. This case report highlights a 42-year-old skateboarder who sustained this rare injury and underwent open reduction and internal fixation with both anterior and posterior plating, offering unique insights into managing complex ankle injuries.
